• Current illnesses : A record of all known and suspected active health problems.
• Past illnesses : A record of all known health problems in the past
• Allergies : Information about drugs, foods, substances, pants, animals and others that cause allergic reactions.
• Immunizations : details of received vaccinations
• History of hospitalizations : number of previous hospitalization and details of each hospitalization including place, time, causes, diagnosis, investigation, and treatment
• History of surgeries, operations, and other medical procedures : number of previous procedures and details of each procedure including outpatient setting or inpatient setting, places (clinic or ambulatory center or hospital), time, causes, diagnosis, results, and even complications
• Medications : A list of current and past medications, including generic or brand names of medications, routes, dosages, frequency, and duration
• Other relevant information : A record of previous accidents, injuries, or any other significant events related to health.
